Instructions and Notes
- Sometimes online forms flake out. You can try this test form to see if online forms are working.
- The evaluation comes in 5 parts. This is to give you convenience, flexibility, and protection against computer crashes:
- You don't have to complete all 5 parts at once.
- You can complete the 5 parts in any order.
- If you make a mistake on a part or the computer crashes,
you need to resubmit only that part, not the entire evaluation.
- You may resubmit any part any number of times. We will simply use your last submission of each part.
- For this arrangement to work, you must use the same ID on all 5 parts. We will not process your evaluation unless you do so.
- Do not use your NetID or CUID, even if you don't care about anonymity.
- Use our ID-generator to obtain a random, anonymous, 7-digit ID. Be sure to save (e.g. write down and/or e-mail it to yourself) your anonymous ID for use on all 5 parts. Don't throw it away until after you have successfully completed all 5 parts.
- This online evaluation has been carefully designed. It does not secretly gather any information. The only information gathered is the time and date of submission and the information that you expressly enter by keyboard or mouse. We have used similar forms past semesters to great effect: We received a lot of useful, anonymous information that we used to reshape the course.
- To receive credit for completing this evaluation, you will non-anonymously fill out a separate Certification form to assert that you completed all 5 parts of this evaluation. To prevent your anonymous Evaluation from being linked with your non-anonymous Certification submission, the Certification form will not be available until after this evaluation has been de-activated.
- Besides the anonymous ID, anonymity comes from safety in numbers: With this many students, there are many students who will give similar answers, thereby providing anonymity.
- We ask you to identify your section instructor so that we can evaluate the effectiveness of each section instructor. Each section instructor has many students, so even if some of their sections are small, anonymity is still provided. However, in case you're uncomfortable identifying your section instructor, we allow you to select "Choose Not to Answer" as your section instructor.
- We ask that you provide your prelim score: It is important for us to have these scores so that we can compute correlations with different aspects of the course to evaluate their impact on performance. Note that we have mechanisms in place to provide anonymity: we ask you to round your scores to the closest multiple of 5 and to report scores below 50 as if your score was 50.

If you haven't already done so, get and save your random, anonymous, 7-digit ID.
If you have already gotten and saved your anonymous ID,
then you can do the 5 parts of the Evaluation in any order.
