This survey is based on the standard Engineering survey but has some additional answers and questions, e.g. we've sometimes enhanced the (1)-to-(5) scale with additional entries (-1) and (-2). We estimate Questions 1-to-17 will take you about 10 minutes. How much time you spend on essay Questions 18-to-20 is up to you, but we do appreciate constructive criticism. Note: please use return and enter to separate paragraphs but not for word-wrap; although the Confirmation page generated when you press Submit does not show word-wrap, please be assured that we receive paragraphs properly wrapped.
This HTML form has been carefully designed. It does not secretly gather any information. The only information gathered is the date and the information that you expressly enter by keyboard or mouse. We used a very similar form last semester to great effect: We received a lot of useful, anonymous information that we used to reshape the course.
Question 0 asks for an ID. This is required: We will not process a form without an ID. To preserve anonymity, we have included a generator of random, anonymous IDs. However, if you don't mind revealing your identity or wish to do so, e.g. you would like us to e-mail a response to you, then you may enter your NetID.
If you make a mistake, please resubmit the form using the same ID.
Anonymity comes from safety in numbers: With this many students, there are many students who will give similar answers on the survey, thereby preserving anonymity.
We ask you to identify your section instructor so that we can evaluate the effectiveness of each section instructor. Most section instructors have many students, even if some of their sections are small, so anonymity should be preserved. However, in case you're uncomfortable identifying your section instructor, we allow you to select "Choose Not to Answer" as your section instructor.
We ask that you provide your prelim scores: It is important for us to have those scores so that we can compute correlations with different aspects of the course to evaluate their impact on performance. Note that we have mechanisms in place to help preserve anonymity, e.g. we ask that you round your scores to the closest multiple of 5.
