It is helpful to make a detailed sequence of events list/chart to check your design.   Here is the sequence of events for when one application dials another.  The Gateway is a special kind of application and has a slightly modified desktop signaling component.  Furthermore it has a private protocol with the PBX server which is used to call into the PSTN.  Here is an outline of what happens when an application calls a real telephone number.
